General Summary:
Postgraduate Year One (PGY1) pharmacist resident is a pharmacist in training who provides comprehensive pharmaceutical care under the supervision of preceptors. The resident will provide a level of service consistent with expectations of a clinical pharmacist and patient care pharmacist.
Responsibilities:
1. Medication/Solution Order Review - Reviews prescribers orders for appropriateness relative to the patients condition, height, weight, and age. Review prescribers orders for appropriateness and compliance with legal requirements, institutional policies and procedures, and drug use restrictions in accordance with prescribing limits set forth by the Pharmacy and Therapeutics Committee and the Medical Executive Committee.
2. Medication/Solution Order Processing Utilizes the full functionality, which includes allergy review, duplicate therapy, drug-drug interaction, dosage range, IV compatibility and food-drug interaction screening of the automated order entry system to facilitate accurate and timely medication/solution distribution and administration. Documents interventions using the approved documentation tools. (Pharmacy Information System Competency)
3. Technician and Ancillary Staff Supervision Reviews medications prepared for distribution by the technicians with respect to accuracy, appropriateness, and timeliness. Monitors technician accountability to follow established policies and procedures for medication delivery, including the automated distribution system. Reviews technician preparation of intravenous admixtures, parenteral nutrition compounding, chemotherapy and miscellaneous sterile products.
4. Clinical Responsibilities Maintenance of daily clinical logs with a focus on continuity of care for the monitoring and documentation of the patients assigned to the Pharmacy Consult services for Parenteral Nutrition; Pharmacokinetics; warfarin, etc. Responsible for the education of patients about their drug therapy. Interpret laboratory data in context with medication use. Determine alternative therapies for achieving outcomes.
5. Drug Information Resource Provides pertinent drug information to medical and ancillary hospital staff. Researches medication dosages, indications, interactions, compatibility, and administration when requested by healthcare professionals. Authors Pharmacy newsletter articles and conducts staff development and educational services to pharmacy staff, healthcare professionals and our community (via outreach programs). Conducts formal Continuing Education presentations to the Department of Pharmacy.
6. Education Resource - Conducts staff development programs and provides educational service to pharmacy staff, pharmacy students and residents; nursing staff, physicians, and other healthcare professionals. Answers drug information questions upon request by pharmacists and other health care professionals.
7. Departmental Performance Improvement Participation Participates in departmental performance improvement by ensuring that all quality control measures are followed and properly documented. This includes, but is not limited to: monitors for Automated Distribution Cabinet Report Reviews, e.g. Override and Discrepancies; identification of medication use scenarios which require submission of Safety First Reports.
8. Follow the ASHP-Residency guidelines, as approved by Bayhealth Completion of core learning rotations, as outlined in the Residency Manual. Achievement of ASHP goals and objectives as outlined in the Residency Manual. This includes a poster presentation at a national conference, and a presentation at a regional conference (Eastern States that meets Bayhealth standards. Fully participates in the Education Certificate Program, attends all required lectures, and completes all work necessary to achieve certificate (if applicable.
